By using the My Account tab, parents of Winning Smiles patients can access and complete forms online, prior to appointments, allowing Dr. Erin to review your child’s medical history and letting the office check dental insurance benefits while cutting down on paper.  Not comfortable filling out paperwork online?  Rest assured that all info is sent over a secure network directly to the office.  However if you’d prefer to fill out forms at the office, or if you have trouble with the online version, please contact us for further assistance.  We're here to help!
